The Enhanced Care Management (ECM) Program, under Fred Finch CARES, provides a comprehensive care coordination service to children, youth, and families, launched in July 2023 as part of CalAIM (California Advancing and Innovating Medi-Cal). ECM follows a Whole Person Care Approach, offering services that address physical, behavioral, emotional, and social health needs, ensuring better care coordination and improved health outcomes. Available to families insured through Managed Medi-Cal, ECM operates across both Northern and Southern California, aligning services across multiple systems of health and behavioral healthcare while addressing social determinants.

The program is available to:

Families with children, teens, or young adults up to 21 years old

Youth up to 26 years old if involved in the Foster Care System

Children, teens, or young adults enrolled in the CCS/CCS Whole Child Model

Children and families at risk of or experiencing homelessness

Children with mental health or substance use conditions
Caregivers of children and youth in these categories are also eligible to receive support.

Our Enhanced Care Management program offers free, personalized support to help you navigate life's challenges, whether in person or via telehealth. We provide:

One-on-One Guidance: A dedicated care coordinator to help you communicate with and track all your healthcare providers.

Access to Services: Assistance in finding doctors, dentists, therapists, and scheduling appointments.

Community Connections: Support in connecting to local programs and resources.

Advocacy and Assistance: Help with food, transportation, housing, or other basic needs.
Through a thorough assessment that includes the Social Determinants of Health, ECM tailors services to meet each family’s unique needs. Families are supported by care coordination, outreach and engagement, health promotion, and advocacy, and are linked to necessary services and resources to improve overall health and stability. Each family is assigned a dedicated care manager who works with them every step of the way.

Families with children or individuals up to the age of 21-year-old (or up to 26 years old with a history of foster care) under a Managed Care Medi-Cal plan are eligible to participate in ECM at no cost

Once in the program, a Fred Finch Care Manager will meet with you in the office or in the community

Referrals are primarily made through Medi-Cal managed care plans. All approvals for services must come through these plans.
